Includes coordination of referrals and patient care related supervisory functions. Consults and collaborates with Vice President of Hospice Services to facilitate the development, implementation and monitoring of strategic plans, goals and objectives.ROLE OVERVIEWKnowledge and Experience

          • Communicates within and outside of the System, including partners, customers, or vendors.ESSENTIAL DUTIES AND RESPONSIBILITIES
            • Provides leadership, direction and supervision for Administrators for Hospice Services.
            • Assumes role and responsibilities of the Administrator for Hospice Services during absences as needed.
            • Implements Hospice goals and objectives under the direction of the Vice President of Hospice Services in accordance with the mission of Hospice Services.
            • Participates in the identification of the need for, and assists in the coordination of the development of services and best practices.
            • Oversees compliance of Quality Assurance Performance Improvement Program
              • Includes monthly targeted record reviews
              • Quarterly Hospice Service audit/office visits in between focus on areas of opportunity
              • Quarterly evaluation of program integrity, rollup of QA data and evaluation of results
              • Identifies trends in opportunities and develops education related to areas of improvement
              • Participates in QAPI meetings and develops a plan of action for improvement needs
              • Evaluates financial performance through variance reports for each Hospice Services Location to determine opportunities and needs for the location Administrator.
              • Monitors clinical outcomes (HIS/CAHPS) and performance metrics and implements education related to new clinical practices or identified opportunities.
              • Develops and implements Quality Assurance Annual Reviews to be completed annually for each location and follow up plans.
              • Provides on-site survey support during certification or complaint surveys.
              • Provides support and direction for Administrators related to staffing and personnel concerns, aides in interview process when appropriate.
              • Reviews and updates organizational forms and tools utilized by the hospice for documentation.
              • Provides training and education to associates on Electronic Health Record.
              • Monitor census/outreach reports and provides review and guidance for outreach and implements action plans in conjunction with the Administrators when opportunities occur.
              • Serves on appointed System project teams or persona groups.
              • Ensures that efficient systems, procedures, and controls are established for all clinical operations.
              • Works closely with the Administrator and Medical Director for Hospice Services to plan and implement policies and procedures relative to clinical operations and set standards for all performance objectives.
              • Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
              • Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
              • Performs other duties as necessary to ensure the success of the System.SKILLS AND ABILITIES
                • Requires strong skills in the area of hospice clinical operations, knowledge of state and federal hospice care rules and regulations, and management team development.
                • Effective communication and delegation skills are required.
                • Must accept responsibility for ensuring that the team has appropriate clinical practice skills appropriate for the direction and supervision of hospice services.
                • Ability to travel throughout the areas serviced by Hospice Services.
                • Thorough knowledge of management and organizational concepts.
                • Clearly understands the organizational structure.
                • Considerable knowledge of laws, rules, and regulations governing human resources management.
                • Ability to effectively communicate and present information in a clear and concise manner, both orally and in writing.
                • Ability to establish and maintain effective working relationships with associates, department heads, administrators, member organization presidents and their leadership teams, and the general public.MINIMUM QUALIFICATIONS
                  • Registered Nurse, currently licensed in the State of Georgia preferred.
                  • Minimum of three years Hospice experience in a leadership position.
                  • BSN preferred.SUPERVISORY RESPONSIBILITIESYesLANGUAGE SKILLSAbility to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or associates of the organization.MATHEMATICAL SKILLSAbility to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.REASONING ABILITYAbility to apply common sense understanding to carry out instructions furnished in written, oral or diagram format.
