Management Position Overview: We are seeking a Registered Nurse
Case Manager for a full-time contract role at a regional office in
Atlanta, GA. This position is onsite and supports the Quality Risk
Management & Patient Transition team . The RN will be responsible
for clinical assessments, care coordination, patient advocacy, and
ensuring transitions of care are safe and efficient. Key
Responsibilities: Conduct clinical assessments to determine
appropriate levels of care and transition planning Coordinate
services and resources across the care continuum Collaborate with
interdisciplinary teams to ensure patient needs are met Monitor
progress and update care plans in accordance with clinical
protocols Advocate for patient-centered care and timely service
delivery 27840852EXPPLAT Job Requirements Requirements: Active RN
license – Georgia (or compact) license required BLS certification –
Required Minimum 2 years of experience in case management,
discharge planning, or transitional care preferred Strong
communication, organizational, and EMR documentation skills
