Job Overview: Under the guidance of the primary care operations
team, the Primary Care Patient Care LPN collaborates closely with
our Primary Care Providers to deliver safe, timely, effective, and
patient-centered care. This role emphasizes systematic approaches
to ensure equitable healthcare outcomes. The Primary Care Patient
Care LPN will work alongside patients, healthcare providers, and
other clinical team members to enhance the quality of care for all
individuals. Responsibilities will be executed in alignment with
our organizational mission, values, and service standards. Position
Information: Essential Qualifications: - Education: Completion of
an accredited practical nursing program. - Licensure: Must hold a
valid LPN (Licensed Practical Nurse) license in the state of
employment. - Preferred Experience: At least 6 months of clinical
experience in a healthcare setting, ideally within a primary care
environment. Additional Skills and Attributes: Ability to
collaborate effectively as part of a team and engage with all
members of the care team. Strong written and verbal communication
abilities. Customer service-focused mindset. Exceptional
organizational and interpersonal skills. Adaptable and positive
demeanor. Keen attention to detail and accuracy. Self-motivated
with the capability to perform tasks independently within the scope
of practice with minimal supervision. Physical Demands: Regularly
required to push, pull, or lift up to 50 pounds. Must be able to
stand and walk for extended periods during each shift. Ability to
grip, reach, bend, kneel, twist, and squat as necessary to fulfill
