Provide direct nursing care in accordance with the patient's care plan Communicate effectively with patients and their family members, ensuring comfort and safety at all times Monitor patient health status and report significant changes to a supervising RN or doctor Administer medications accurately and safely, and maintain detailed records of patients’ treatment plans and medical history Assist with daily living activities, such as bathing, dressing, and eating Educate patients and their families on health care, including managing illness or injury and preventive measures Coordinate with other healthcare team members to optimize patient care across multiple disciplines Strong clinical skills in a home healthcare setting Excellent communication and interpersonal skills Critical thinking and problem-solving Compassion and high level of empathy Detail-oriented with strong documentation skills Medical Insurance Dental Insurance Vision Insurance Life Insurance Current LPN license in the state of practice CPR certified Valid driver’s license and reliable transportation
