What you will do in this role:

  • Assesses and reassesses patient’s condition, completes admission review and begins discharge planning on assigned patients utilizing approved criteria for determining medical necessity.
  • Coordinates plan of care to include identified clinical, psychosocial and discharge needs and communicates to appropriate disciplines.
  • Evaluates and assesses observation patient for appropriateness and facilitates timely transition to next level of care.
  • Performs utilization review functions and communicates with third party payors.
  • Demonstrates knowledge of regulatory requirements, HCA ethics and compliance policies and quality initiatives; monitors self-compliance and implements process change to ensure compliance as it relates to the provision of case management services.
  • Demonstrates knowledge of community resources and referral policies/procedures.
  • Documents case management activities to effectively communicate to all members of the healthcare team.

 

What qualifications you may need:

  • RN with current Georgia licensure or eNCL, BSN preferred
  • Minimum three years nursing experience in acute care setting.
  • Ability to establish and maintain collaborative and effective working relationships
  • Ability to communicate effectively in oral, written and electronic formats
  • Demonstrates critical thinking skills to ensure proactive decision making and negotiation skills.
  • Demonstrates basic knowledge of the growth, development and range of treatment required by assigned case group.
  • Demonstrates willingness to accept responsibility and accountability in monitoring the quality of patient care.
