Company DescriptionOur organization is seeking an Independent Living Program Coordinator - Independent Living Service Delivery, Assistive Technology, Durable Medical Equipment Reuse Program and Management of Section 8 Referral Program. This position will report directly to the Executive Director. This person must be exceedingly well organized, highly flexible and enjoy challenges. The role is challenging but will provide a wide variety of experiences. Our organization's Executive Director highly values proactiveness and is looking for someone eager to learn and take on ever increasing responsibilities. It's an exciting role, no day is the same. Dedication, great communication skills, optimism, and a sense of humor towards life and work are essential. IL Philosophy

  • Visits with individuals with disabilities in their homes, nursing facilities a public place, or in the office as mutually arranged. Contact your open consumers (individuals with a disability) regularly, at least once a month. Shares life experiences and offers encouragement, serves as a role model.
  • Promotes empowerment through individual advocacy efforts.
  • Listens and facilitates self-assessment by the consumer which results in an action independent living plan directed by the consumer.
  • Coordinate with Executive Director to fulfill short-term projects/events and initiatives as requested by the Executive Director.
  • Prioritize with the Executive Director and manage multiple projects simultaneously and follow through on issues in a timely manner - use and become proficient with computer-based applications for project and task management.
  • Respect the need for confidentiality and sensitivity of information.
  • Have a proven ability to exercise good judgment in recognizing the scope of authority and appropriate professional attitude towards organization mission and environment.Listening skills
    • Listen and understand the consumer's issues
    • Let the consumer speak without interruptions, however, when you do have tointerrupt, do it as respectfully as possible. (Do not hold a grudge or exhibit frustration to the consumer - verbal or nonverbal.)
    • Be sure to listen first before offering "fixes" for the issue or situation. (Remember that it is the consumer's choice to heed or accept advice and suggestions.)Customer service
      • Know what you can and cannot do for the consumer
      • Know your boundaries/parameters - it's ok
      • Be gentle with the things we don't do or have waiting list for, but assist with researching to find options
      • Even though consumers may be placed on waiting lists, think proactively how you can assist the consumer with other current needs and options
      • Inquire about issues filling out applications, be considerate of possible sensory, learning, mobility disabilities, literacy and transportation barriers. Offer to provide assistance with our applications and other community resource applications.Consumer Service Records
        • must be maintained which includes Demographics, Eligibility Forms, Voter Registering offering, Independent Living Plan or Waiver, uploaded all consumer related documents and all contact activity records must be documented.
          • All Community Activities must be documented including all activity not associated with individual consumers as it relates to organizational goals and duties assigned by the Executive DirectorQualificationsESSENTIAL REQUIREMENTS
            • Bachelor's degree or equivalency in experience preferred
            • Knowledge and experience with assistive technology and durable medical equipment
            • Experience customer service / case management a plus
            • Excellent attention to detail, accuracy and quality
            • Excellent written and verbal English communication skills
            • Ability to maintain calendars and schedule appointments
            • Ability to compose and edit written materials
            • Ability to work easily and effectively with a wide range of people
            • Able to manage multiple projects and responsibilities at once; ability to meet deadlines
            • Ability to be flexible and exercise sound judgment
            • Ability to work flexible hours and on weekends as needed
            • Possesses time/organization/stress management skills
            • Strong commitment to our goals and mission
            • A responsible attitude to all aspects of the work
            • Demonstrates solid work ethics
            • Proficiency with MS Office Suite especially Word, Excel and PowerPoint - Mac Based systems like Keynote, Pages and have an acuity for using and learning various databases and software systems.
            • Must have reliable transportation to serve 12 county area (mileage reimbursement is provided)
